All participants that sign up for the Summer Camp / League, must be a member of the US Field Hockey Association. If you are not a member, you must print out the following two forms, and submit to the US Field Hockey association to get your membership number. You will need your membership number, and expiration date for the Summer Camp / League registration forms

(Girls Presently in grades 5-7)

 

The Following forms need to be filled out for Camp Registration (Girls presently in grades 5-7)

Summer League

(Girls Presently in grades 8-11)

 

The Following forms need to be filled out for League Registration (Girls presently in grades 8-11)

If you are submitting a whole team, your must include the team Roster Registration form also

You Must register by June 8th.
